Just this year, the American College of Gastroenterology published an evidence-based systematic review on the management of irritable bowel syndrome (IBS). These guidelines, which were prepared by a broad-based task force, represent the state of the art in terms of diagnostic criteria, the value of diagnostic tests, and the efficacy of therapy in IBS. Nevertheless-like all guidelines-there remains considerable room to interpret the recommendations, and the practicing physicians has substantial latitude in putting these guidelines into practice.
Together with Dr. Lin Chang, Dr. William Chey, Dr. Mark Pimentel, and Dr. Philip Schoenfeld, we will follow a "Trial by Jury" approach, in which we will put the recommendations for 3 classes of therapy for IBS - including probiotics, antibiotics, and antidepressants - on trial. Using this format, we will first determine whether the evidence truly supports the recommendaiton and second, we will examine how to optimally use these drugs in clinical practice. We believe this format allows us to explore the considerable controversies that still exist regarding the use of these therapies in day-to-day practice. Hope to see you at the trial!
